Protected Health Information (PHI)

This website’s forms are for business inquiries only and are not intended to receive patient information. Please do not submit patient names, medical records, or any other protected health information through this site. When RevPath performs billing services for a client practice, PHI is handled under a signed Business Associate Agreement (BAA) inside HIPAA-compliant systems — separate from this website.

How We Use Your Information

  • To prepare for a consultation or billing audit you requested.
  • To operate, secure, and improve the website.
  • To comply with legal obligations.

We do not sell your personal information, and we do not share it with third parties for their marketing. Form submissions are delivered to our team by email and stored in our website’s form system.

Service Providers

We use a small number of infrastructure providers to run this site — web hosting, email delivery, and site security/caching. These providers process data only as needed to provide their service to us.

Data Retention & Security

We keep inquiry data for as long as needed to handle your request and maintain our business records, then delete it. Access to submitted data is limited to RevPath team members who need it, and the site is served over HTTPS.
